Collection of Personal Information and Cancellation Policies.

COLLECTION OF PERSONAL INFORMATION

PRIVACY ACT 1988, THE PRIVACY AMENDENT (PRIVATE SECTOR) ACT 2000.

We require your consent to collect personal information about you and your child. Please read this information carefully, and sign where indicated on your Patient Information form.

Stepping Forward Therapy Services collects information from you for the primary purpose of providing quality health care to your child. We require you to provide us with you and your child’s personal details and a full medical history of your child so that we may properly assess, diagnose and treat illnesses and be pro-active in your child’s health care.

This information is private and confidential and is stored securely. All staff members at Stepping Forward Therapy Services are familiar with the key aspects of privacy legislation and the Australian Physiotherapy Association’s Code of Conduct to ensure they manage your information appropriately.

Stepping Forward Therapy Services has a Privacy Policy that is available upon request and can be found online at www.steppingforward.com.au. This policy provides guidelines on the collection, use, disclosure and security of your child’s information.

We will also use the information you provide in the following ways:

  • Administrative purposes in running our practice, including billing purposes, compliance with Medicare and Health Insurance Commission requirements.
  • Relevant disclosure to others involved in your health care, including treating doctors and specialists outside this practice. This may include disclosure of information your child’s carer, integration aide or school if this is the location of therapy and/or involvement has been requested.
  • Disclosure to other physiotherapists in the practice, for the purpose of patient care, teaching or handover. Please let us know if you do not want your records accessed for this purpose, and we will note your record accordingly.

I have read the information above and understand the reasons why my information must be collected.

I understand that I am not obliged to provide any information requested of me, but by not doing so may compromise the quality of the health care and treatment given to my child.

I am aware of my right to access the information collected about my child, except in some circumstances where access might legitimately be withheld. I understand I will be given an explanation in these circumstances. I understand that if I request access to information about my child, the practice is entitled to charge me fees for (i) the time spent by administrative staff to provide access; (ii) the time necessarily spent by a medical practitioner to provide access; and (iii) for photocopying and other disbursements.

I understand that if my information is to be used for any other purpose other than set out above, my further consent will be obtained.

I consent to the handling of my information by this practice for the purposes set out above, subject to any limitations on access or disclosure that I notify this practice of.

 

FEES AND CANCELLATION POLICY

Stepping Forward Therapy Services is committed to providing children and their families with exceptional care. Treatment goals and sessions are planned and prepared for by your physiotherapist prior to appointments for the benefit of your child. When an appointment is cancelled at short notice, or a child does not attend their appointment, another child is prevented from being seen. Our cancellation policy assists in minimizing this unfortunate circumstance.

Please call us on 03 9899 4004 by 3:00 pm on the day prior to your scheduled appointmentto notify us of any changes or cancellations.

If insufficient notification is given, cancellation fees apply as follows: Clinic appointments – 50% of the appointment fee, Community appointments/NDIS funded/Home visits – 100% of the appointment and travel fee.

Sometimes there are unforeseen events. If your child is unwell on the day of the appointment, or your circumstances suddenly change, please call us on 03 9899 4004 as early as possible to notify us so we can reschedule the appointment for you. Alternatively, you can reply to your SMS reminder.

Payment of invoices are due within 7 days of issue. Should this account be outstanding after 7 days, a late payment fee of $15.00 will apply and $15.00 for every month past the due date.
